The price, before the pen

When a prescriber signs a medication order, EveryCard Rx shows what it will cost this patient under their own plan, and the cheapest way to fill the same medicine.

Patient pays about $248.70 this fill on Wellcare Classic (PDP) (tier 3, limit of 74 per 30 days).

Wellcare Classic (PDP) (S4802-083): tier 3; limit of 74 per 30 days.

This fill: about $248.70 at a preferred pharmacy. Deductible: the patient pays the plan's full price, $248.70, and $366.30 of the deductible remains.

Rest of 2026: about $497.40 over 2 fills.

A full 2027, if the plan's benefits stay the same: about $1,333.33.

Estimate from CMS's Part D plan files; the patient's spending so far this year isn't known here, so the deductible is assumed unmet.

Lowest cash price for the same medicine near 337xx: $341.17 at Walmart Pharmacy (savings card). Cash fills don't count toward the Part D yearly cap.

Bristol Myers Squibb Patient Assistance Foundation may give Eliquis free to patients under its income limit. The patient applies; the prescriber completes one section.

Demo: some savings-card prices are estimates.

Same medicine only: EveryCard Rx doesn't suggest other drugs.

What the health record sends and receives

The record calls POST https://everycardrx.com/cds-services/everycard-rx-cost on order-sign (and everycard-rx-cost-select on order-select), with the patient and their coverage prefetched as the discovery document asks. This is the exchange behind the card above.

The same service, added by its discovery URL to sandbox.cds-hooks.org and called when its Rx Sign screen signs an order for atorvastatin 20 mg.

Connect a health record

EveryCard Rx is a CDS Hooks 2.0 service on the order-select and order-sign hooks. It reads the draft MedicationRequest (by RxNorm code, or its name), the patient's ZIP and their active Coverage; a Medicare plan ID in the coverage gives the plan's own cost.

Discovery: https://everycardrx.com/cds-services

Nothing about the patient is stored or logged, and the cards only ever name the same medicine: choosing a different drug stays the prescriber's call.
