Who can apply?
| Household | 1 person | 2 people | 3 people | 4 people |
|---|---|---|---|---|
| Yearly income up to | $47,880 | $64,920 | $81,960 | $99,000 |
Annual pre-tax household income at or below 300% FPL for all products ($47,880 for 1 in 2026). Older pages citing 400-600% FPL are outdated.
Commercially insured patients are not eligible. Uninsured and government-insured patients (the page names Medicaid, VA and TRICARE as examples of government insurance) may qualify if they cannot afford their copay. Medicare Part D/Advantage patients must enroll in the Medicare Prescription Payment Plan and send proof if income is above 138% FPL, attest to the $2,100 out-of-pocket maximum, and apply to all applicable independent charitable assistance programs (copay foundations) and send proof of denial. Must apply to applicable alternate funding resources first. Requires a valid on-label prescription.
